Camper Eligibility:
- The child’s illness or condition must interfere with their ability to attend other non-medical camps.
- Children must be ages 7 through 16 while attending camp.
- Children must be within three years of active treatment and/or currently experiencing the challenges of a chronic or life-threatening illness.
- All children attending camp must be fully immunized, unless an immunization is not medically advised, in which case you must submit a medical exemption signed by your physician.
- Children must have social skills appropriate for camp; not exhibiting challenging behavior which warrant removal from a group activity or setting.
- The child must have the physical and emotional ability to actively participate in our camp programs.
All applications will be reviewed and considered individually by our Medical and Camp Director. Decisions are made based on the child’s inability to attend non-medical camps and the severity of the child’s medical issues.
The Application Process
Camp Boggy Creek has an extensive multi-step application process to ensure that all of your child’s needs are met at camp. In order for your child’s application to be reviewed you will need to complete and submit the following:

Complete the Online Camper Application
If you do not have access to the Online Application, please contact your Camper Recruiter to mail you an application,
Once we receive your Online Camper Application, we will send you the following:
Camp Medical Form
Disease Specific Medical Form

| Step 2: |
Have your child’s specialist and/or primary care physician complete and sign the Camp Medical Form and the Disease Specific Medical Form. Return the signed forms to us.

Steps 1 – 2 must be completed entirely before your child’s application can be reviewed. |

You will be notified by Camp Boggy Creek of your child’s acceptance status by the timeline listed above. Please remember, even though we try to accommodate every child who applies, a submitted application or an online application reservation acknowledgement does not guarantee acceptance. Your Doctor, Nurse or Foundation cannot promise acceptance. |
